Little Rock, Arkansas – September 2019 Harmony Park Family Medicine in Hot Springs, Arkansas has joined SHARE, the only statewide health information exchange (HIE). Harmony Park Family Medicine is part of the continued connection of independent family medicine practices to join the SHARE network!
Harmony Park Family Medicine is providing high quality, patient-centered care to include: adolescent and adults care, prenatal care, children, same day visits, after hours’ care and non-emergent questions.
Access to SHARE will give the Harmony Park care team secure access to important health information such as; continuity of care documents (CCDs), laboratory results, updated patient records, discharge summaries, medication histories and demographic information of their patient’s. This ensures every care provider in the patient’s chain of care has consistent and current information, and that their health records are more secure and accessible.
The Harmony Park Clinic will be sharing data through an interface with NexGen electronic medical record (EMR) which features a robust patient query and response function that will allow providers to quickly access patient health data from hospitals, physician practices, state health systems and more.
The Harmony Park care team will be sharing patient information like; medical history, previous diagnoses, lab tests, medications, continuity of care documents (CCDs), immunizations, vitals and allergies. The clinics will also send their immunizations through SHARE to the Arkansas Department of Health, immunization registry (WEBIZ). This will improve their workflow and give them access to a bi-directional immunization interface.
SHARE will provide Harmony Park with access to 24-hour daily reports notifying their providers and care team when their patients are admitted and discharged from the emergency department and inpatient discharges.
As the health care landscape continues to transform, HIEs continue to play a key role in providing data that fuels innovation. SHARE and Harmony Park are working together to contribute to these innovation efforts such as population health initiatives, assisting with high-risk patients and identifying potential social determinants of health.
