SHARE New User Request

You must be a Trusted Agent to Request a SHARE New User.

Each participating organization has an assigned person as a Trusted Agent. The Trusted Agent plays a vital role during the implementation of the SHARE systems within your organization and, most importantly, in maintaining and managing the security of patient health data information. Trusted Agents manage Authorized Users and serve as the central point of contact for the SHARE Support Team. If you do not know who your Trusted Agent is, you can call 855-742-7327 or email [email protected] and request the information.

Forms filled out by non Trusted Agents or incomplete forms can not be processed.
