Being notified by SHARE’s daily hospitalization reports is crucial for Arisa Health as it provides timely updates on their clients’ hospitalizations. This information allows Arisa Health to promptly follow up with clients upon discharge, ensuring continuity of care, maintaining the gains from inpatient care and the risk of rehospitalization. The resulting community tenure improves the client’s quality of life. Additionally, these reports are enabling Arisa Health to coordinate care with hospitals, other providers, and payers, ensuring that all involved are working together for the client’s benefit. By identifying clients at higher risk for hospitalization early on, Arisa Health will be able to intervene proactively to mitigate these risks and prevent future hospitalizations. Moreover, the data from these reports can be analyzed to identify trends and areas for improvement in care processes, ultimately leading to better outcomes for patients.

For example, when a client receives behavioral health services at Arisa Health, their PCP will be able to easily access relevant information about their treatment plan and progress, enabling more informed decision-making and better coordination of care between the two providers. As we gain access to a more comprehensive view of the client’s health history through SHARE HIE Arisa Health aspires to make more informed decisions about the client’s care. For example, if a client is prescribed medication for a mental health condition at Arisa Health, their PCP can now see this information and avoid prescribing medications that may interact negatively with the client’s existing medications, improving client safety.
The integration with SHARE HIE holds the promise of streamlining workflows at Arisa Health, reducing the time and effort required to access and share patient information. This desired efficiency gain will allow Arisa Health to focus more on providing direct care to clients and less on administrative tasks, ultimately improving the overall client experience.
