Being a primary care physician means knowing the most current information regarding your patients. SHARE daily reports is helping Gray Family Practice Clinic do that. The daily reports make available to their health care team the most recent critical events of their patients within the last 24 hours regardless of their insurance payers.
The State Health Alliance for Records Exchange (SHARE), aggregates health data from many major health systems across the state of Arkansas. By doing this the health information exchange (HIE) is designed to reduce instances of redundant care by ensuring providers have a complete, holistic view of patient care when making clinical decisions.
The 24-hour daily reports are a data service that make updates available to Gray Family Practice Clinic’s team when a patient is:
- Admitted to an emergency department
- Discharged from an emergency department
- Transferred from outpatient to inpatient
- Discharged from an inpatient encounter
Access to this timely information can be used to:
- Improve care management and coordination for patients or members;
- Prevent hospital readmissions;
- Improve disease management and inform quality improvement;
- Help health care providers pinpoint areas for improving outcomes and assist in containing costs.
The Gray Family Practice Clinic’s care team is also accessing SHARE’s virtual health record (VHR), that aggregates patients records outside of their system of NexGen electronic medical record (EMR) system enabling more accurate diagnoses, faster and more effective treatment, and better outcomes for the patient.
The clinic can now view the following historical patient data through SHARE’s VHR:
- Admission, discharge and transfer reports
- Allergies, problems, medications, demographic and insurance information
- Laboratory results, radiology reports, image links
- Discharge summaries, continuity of care documents (CCDs), progress notes and vitals
About Gray Family Practice Clinic:
Gray Family Practice Clinic, PLLC officially opened on June 28, 2012. Dr. Adam C. Gray is the owner and Medical Director. Fern Sherrell, a life-long resident of Izard County, is the Nurse Practitioner on staff. They evaluate and treat a wide variety of age groups, from infants to the elderly. They also provide services for yearly wellness exams, such as annual Medicare preventative visits, pap smears, breast exams, and prostate checks. Their clinic was built with on-site digital x-ray equipment as well as a CLIA waived lab. Simple tests can be run in the Clinic, while more complex tests must be sent to a reference lab. Walk-ins are gladly accepted, but appointments are preferred.
Here is a short list of medical conditions that are treated on a regular basis in the clinic:

- High blood pressure
- Thyroid problems
- Anti-coagulation medication oversight
- Weight management
- Attention deficit hyperactivity disorder

Get to know their practitioners:
Dr. Gray graduated from Hendrix College with a Bachelor of Science degree in Chemistry in 1994. He attended the University of Arkansas for Medical Sciences and graduated in 1999. His residency was completed at AHEC in Jonesboro. He then moved to Melbourne and practiced for 10 years in a rural clinic before opening his own practice. Since his time in residency, Dr. Gray has practiced emergency medicine at several hospitals in and around Izard County. He currently works in the Emergency Department at Fulton County Hospital. Dr. Gray has also served as the Medical Director of Pioneer Nursing and Rehabilitation Center in Melbourne and Kidspiration in Mountain Home.
Fern Sherrell began her career in nursing as an RN in 1990. She worked as an RN for several years before becoming an LPN instructor at Ozarka College. She taught at Ozarka College for 6 years and then decided to return to school. She graduated in 2004 from Arkansas State University with a Master of Science in Nursing as a Family Nurse Practitioner. Fern has worked in the health care field for over 23 years and has a variety of experience in the clinical setting, including family practice, pediatrics, and urgent care.
