CHI St. Vincent hospital in Hot Springs expands connection with SHARE to include Continuity of Care Documents (CCDs)- 25+ specialty and primary care clinics expand connections to seamlessly deliver patient health information (laboratory results, vitals, allergies, progress notes, radiology reports and discharge summaries) across Garland County and another fifteen counties in Arkansas.
Little Rock, Arkansas –September 2019 The State Health Alliance for Records Exchange (SHARE) is excited to announce a ground-breaking partnership that will enable more efficient health care services for patients across rural Arkansas. SHARE is Arkansas’ only health information exchange (HIE). This partnership expands connections to CHI St. Vincent Hot Springs by exchanging important information such as lab results, updated patient records and demographic information. This integration with SHARE ensures every care provider in a patient’s chain of care has consistent and current information, and that their records are more secure and accessible.
Enhancing the connection with CHI St. Vincent hospital and clinics in Hot Springs to the statewide HIE network will include bi-directional data exchange, health care providers participating in the clinically integrated network and referral partnerships will have faster and more complete access to their patient’s most critical health information. The enhanced information access will improve patient’s care coordination, reduce treatment delays, eliminate unnecessary testing and allow care providers to make more targeted diagnoses and treatment recommendations.
SHARE provides CHI St. Vincent hospital in Hot Springs with data to enable enhanced 30-day readmission event notifications, as well as 24-hour daily reports when their primary care practice Medicaid patients are admitted and discharged from the emergency department or have an inpatient discharge. This collaboration is to improve population health analysis, patient outcomes and success with health care quality initiatives such as accountable care, promoting interoperability (formally known as meaningful use), clinically integrated networks, MIPS/MACRA and medical home programs.

The data sharing builds a more complete medical record for communities and individuals. With the connection to the HIE, it results in fewer repeated tests, better diagnosis and monitoring of chronic conditions and a more comprehensive patient health record.
Hospitals carry a large proportion of the burden to reduce the cost of care. They must find strategies to reduce or avoid unnecessary hospitalizations and eliminate the over-utilization of emergency room services, cutting costs, and focusing on patients with a truly emergent need. SHARE is excited announces a new partnership with 1st Choice HealthCare sharing continuity of care documents (CCDs) with connection to the eCW Arkansas HUB. 1st Choice Healthcare is a large group of six federally qualified health centers (FQHC), throughout Northeast Arkansas.
